PEPFAR — the President’s Emergency Plan for AIDS Relief — was launched in 2003 and has saved an estimated 25 million lives, mostly in sub-Saharan Africa. In 2025, the Trump administration began dramatically reducing funding for the program, redirecting resources and cutting grants that had sustained a sprawling network of clinics, testing centers, and prevention outreach programs.

The first large-scale survey of affected organizations, published in July 2026, found that 1,700 HIV service sites had closed as a direct result of the cuts. Six large organizations alone accounted for 85 percent of the closures, illustrating how a single funding loss can ripple through an entire health system. Even clinics that remained open reported severe disruptions: shortages of lab commodities, interrupted supplies of condoms and antiretroviral drugs, and the elimination of community outreach programs that had been critical for reaching high-risk populations.

The study surveyed hundreds of organizations that had previously received PEPFAR support. A fifth of them reported stopping at least one service entirely. Five organizations had closed completely, three of them explicitly because they lost their PEPFAR grant. The cutbacks were not limited to treatment: prevention programs, which depend on consistent funding for testing kits, education campaigns, and condom distribution, were hit especially hard.

Global health experts warn that the consequences will compound over time. When prevention programs shut down, new infections go undetected and untreated, leading to more advanced disease and higher transmission rates. The Guardian described the cuts as having “severe and devastating” impacts around the world, while the New York Times reported that the remaining PEPFAR model is now “heavily focused on treatment” but “insufficient for controlling the HIV response.”
