Spicy food is global comfort food. Sichuan hotpot, Mexican moles, Thai curries and Indian vindaloos all owe their kick to the same molecule: capsaicin, the compound that makes chili peppers burn. For years, capsaicin has carried mixed press. Lab studies have hinted at both anti-cancer effects at moderate doses and irritant effects at high ones. A new pooled review of existing studies adds the most thorough dietary-angle evidence yet — and the headline is more nuanced than either extreme.

The analysis, published this summer, combined data across dozens of studies to ask a straightforward question: does how much chili a person eats track with gastrointestinal cancer risk? Across the pooled population, very high chili-pepper intake was associated with roughly a 64% higher risk of gastrointestinal cancers overall. But the signal was not evenly distributed. The strongest and most consistent link was with esophageal cancer: people at the highest consumption levels carried nearly three times the risk of those who ate little or none.

For stomach cancer, the connection was weaker and more variable across studies. For colorectal cancer, the aggregated data showed no reliable association at all. In other words, the risk is real for one specific cancer and far from settled for the others.

Importantly, the researchers stress that the numbers are associations, not proof of cause. Heavy chili consumption clusters with other habits — alcohol use, tobacco, and very hot food and drink — all of which independently damage the esophagus. A diet's "spicy" label also says little about how the pepper is prepared, served, or balanced against the rest of the meal. A chili eaten in an East Asian or Central American context is not the same exposure as a handful eaten at a Western barbecue.

Knowledge takeaway: very high chili-pepper intake is linked in pooled data to a higher risk of esophageal cancer, while the evidence for stomach and colorectal cancer is weaker or absent. The association most likely reflects a mix of the pepper itself, how it is cooked, and the other habits that travel with very spicy diets — so the story is about exposure, not a simple "good versus bad" verdict on spice.
