AI Finds Sperm That Clinicians Missed — First Pregnancy After 19 Years
An AI-guided microfluidic system identified viable sperm in a sample that human microscopes had declared empty, producing the first clinical pregnancy from a man once diagnosed as infertile.

As cells drift past a high-speed camera, an AI model — trained on thousands of images — tags each moving object in real time and steers the fluid so that only the ones flagged as live, intact sperm are funneled into a separate collection channel. There is no laser, no harsh chemical, no surgery. The sperm are simply sorted, gently and automatically.
That matters for men with azoospermia, a condition where a standard semen analysis returns zero sperm and conventional treatment has no obvious next step. Before STAR, the only options were invasive surgical retrieval from the testis or giving up. STAR turns a "negative" result into something far more informative: a tiny, usable population of living sperm that was always there, just invisible to the naked eye and too rare for a technician to scan through in time.
The first-pregnancy case report was published in The Lancet and has since been offered clinically at Columbia. The honest caveat is worth keeping: one couple, seven sperm, one baby so far. STAR is a tool that widens the net, not a cure for the underlying causes of male infertility. But for men who were told definitively "there is nothing to work with," the technology finally gives medicine a way to look harder — and occasionally, to find what was always hiding in plain sight.
